Is there a way that when you add a file it can have a text box appear with a place to put instructions into it for that file and then email it? I am using it as a place where a customer can upload an art file and give me instructions on how to use it. I would like to have it show the file name, a couple of drop downs, and a large text box, for every file added. Would this be to complicated?

for every file?

you can use the description mode for an additional text for each file.
And you can additionally include the flash in a form where ther variables can be sent along with the upload:

Yes for every file....

Do you know if it will work on a form component like RS form, Blue Flame Form? I have those or could you recommend one that would work for this purpose?

JFU does not care which tool you use to create the from. This only thing is that is has to use unique id's for the elements.
The howto does explain how this works.
